All participating non-profit organizations must provide vital services within the Baltimore metropolitan area (Baltimore City and Anne Arundel, Baltimore, Carroll, Harford and Howard Counties). Organizations located outside of the Baltimore metropolitan area may be eligible to participate, if able to demonstrate that their services are provided within this geographical area. Organizations must have carried out its charitable functions for a period of at least one year prior to the campaign’s commencement.

2012 New Agencies

Applications are good for a two-year period beginning in odd years. If your organization did not participate in the 2011 campaign and is interested in being considered for inclusion as a participating agency for the 2012 campaign, please apply beginning February 1, 2012. Please see the list below for documents that are needed for your application.

2012 Participating Agencies

If your organization participated in the 2011 campaign and would like to remain a participant in the 2012 campaign, you do not need to renew an application for 2012. Your organization is considered a Participating Agency and will automatically participate in the 2012 campaign. To report changes, please email Kim Gupton at kim.gupton@baltimorecity.gov.

2013 CCC Campaign

All Agencies must reapply for 2013, whether the organization was considered New or Participating in 2012. If you would like to be included for the 2013 campaign, you will need to complete a new application in February of 2013.

Application Attachment Checklist

Please be prepared to have the following documents ready for your 2012 application:

1. Copy of currently VALID IRS letter designating organization as a 501 (C)(3) agency.
2. SIGNED copy of 2010 IRS Tax Form 990. If your organization does not file a Form 990 or if your 2010 tax form has not been finalized, you must submit a copy of one of the following:
   a. Fiscal ending 2010 audited annual financial statement prepared by a Certified Public Accountant, or;
   b. Fiscal ending 2010 annual financial report approved by the Board of Directors or other governing body and SIGNED and CERTIFIED by the Chief Executive Officer or other responsible person.
3. Copy of currently VALID Articles of Incorporation, by-laws if available, or other governing instrument.
4. Copy of 2012 operating budget SIGNED by the Chief Executive Officer or other responsible person.
5. List of the names and mailing addresses of all members of the organization’s Board of Directors or other governing body.
6. Copy of Certificate or letter from the Maryland Secretary of State showing CURRENT registration as a Charitable Organization. If your organization has a religious exemption, attach a copy of a letter from Maryland Secretary of State showing religious exemption.
7. For organizations whose principal office is located OUTSIDE of the Baltimore metropolitan area, provide EVIDENCE that it has provided services within this geographical area within the past year. Please note that a description of services it provides in this area will not be accepted.
